My Brother received excellent care from the CNA’s, Nurses, Activities Dept, P.T., Dining, housekeeping , Social services, business & admissions dept.My Brother was special needs & staff was very compassionate to his disabilities & health issues. The CNA’s were always so positive and hard working, caring & friendly He was in several Skilled nursing centers & Lake Forest was by far one of the best.Thank you for all you have done. Our family is grateful for all that you did for him

This nursing facility was a wonderful place for my mother to recover from her broken hip. The physical therapy and occupational therapy department is excellent. They were always taking my mom from her room giving her exercise and monitoring her progress. My mom even enjoyed the food! Especially cute is the little ice cream parlor that is open every day at 1 PM and the patients can go and enjoy a treat. They often would have live music on the holidays or for no occasion at all in the dining room. We enjoyed a country music band and a wonderful orchestra with violins and cellos. There is a calendar of daily events that the patients can attend and are encouraged to do so. I am so glad that I did not let the negative reviews on this site turn me away from this skilled nursing facility for my mom’s temporary rehab. Before placing my mom, I went there and saw it for myself and met with the staff. It was clean and cozy and had its own in-house physical and occupational therapy department. My mom even had a private room. I gave it a chance and I am very grateful that I did. My mom is back home now and misses the staff.

My mom was placed here for a month of rehab and recovery after a two week hospital stay, and I can't say enough good things about this facility. Station B - You guys are the best! Always professional, always friendly, always helpful. If something wasn't right, they fixed it immediately, and the physical & occupational therapy she received was excellent! This staff works hard, and we're giving five stars!

Avoid this place at all costs. My mother-in-law was in rehab for a broken hip and while there she had a stroke and no one noticed it for nearly 20 hours. They thought she was tired from physical therapy. When visiting we called their attention to her symptoms and asked to speak to the doctor which they couldn't find and finally 911 was called. She died 24 hours later. We will be reporting them to the state.

My mother was sent her after she was ran over, with rib surgery and a broken arm broken nose. She got there around 4 oclock and she called me around 10pm saying she hasnt received any pain medication or any of her insulin. Also her oxygen tank was over heating 20 min passed and no one showed up till 30 min passed . A nurse named Kat calls my husband complaining that multiple family members were asking for medicine for my mom. She was extremely rude to him and was yelling at him thru the phone. We had to call 911 to get my mom out of there and back to a hospital. Please do not send your family here terrible person non humanitarian workers that don't care about there patients.

Worst place ever!!!Don't send your loved or hated ones to this place.They HIDE the CALL BUTTONS. The contact information in the rooms is incorrect.My 89 yr old mother on Hospice Respite was left in a urine soaked pull up, gown and chair for at least 5 hours.Un organized Un carryingUn professionalUnacceptable This just happened people! I just got her home in worse condition than when she left.

Very poor staff... I think there was 1 RN on staff at any one time..In general they just couldn’t be bothered.... Most of the staff are just low skilled aids or non medical orderlies. Even the televisions didn’t work... I realize this isn’t a hotel, but meals were delivered cold and usually late... Very little consideration for those with nutritional issues... Rude and unaccomodating cafeteria staff.. Felt very prison like, kind of a shut up and we will get to you type of place... Room cleanliness was poor, as there were food scraps sitting on the floor the entire week and a half my father was there...flies etc.. Just a depressing place for your loved one to be...

I don't even want to give this place a 1 star! My grandmother is 86, and has dementia. She has fallen 3 times while staying here. Resulting in a broken hip, the second time she fell. Their solution to her not falling out of the bed (instead of getting bed rails) was to place her in her wheelchair all day at the nurses station with no one talking to her, just sitting and starring. Which isn't helping because she fell again last night. Its sad that if family isn't here to visit her she could just end up sitting in a hall all by herself starring at nothing. Not a good solution to her situation at all! The first week she was here, the one nurse had no clue what the bracelet she was wearing meant. We had to go and ask another nurse. ( It was the type of food she was allowed to have.) Which tells me they aren't trained very well! I feel that they don't communicate with each other, because most of the time they have no clue what's going on! If I could I would remove her from here, but it's not up to me.

Very bad service. My dad was sent here after a stroke and the routines were missed and we kept having to remind people on what he needed on a daily basis. They also forgot to send the correct paper work when he needed his gi tube removed and got canceled. Now a few more days with the tube because someone forgot to send the papers. Very disappointed in management. Put 2 stars because not everyone is bad, a few people were very niceand helpful, but this place lacks good management.

Poor communication from the nursing staff. My mother was there for 5 weeks and we had only one formal meeting with the care team because we went to the ombudsman. My mom lost 15% of her weight in one week and no one mentioned it until my mom did. PT and OT staff was good, but nursing staff was slow to respond to calls, including help to the toilet. The case worker told me they "don't do meetings". Occasionally they stopped us in the hall, and mentioned issues in a very non-HIPPA-compliant manner. They requested her partial dentures be brought in, which they were, then they lost them for 5 days. They misquoted her coverage and said there would be a co pay (also not true), because their stated goal (on the hold recordings) is to get people out in less than 30 days. Medicare pays well for 21, then even with good insurance they want patients out because the negotiated deals pay less. We filed two appeals on mom's discharge but they were both declined (although she'd lost all that weight the we before the discharge). Take your loved one elsewhere! I'd give 0 stars but then there is no way to write a review.
